When a filling is needed and is not done, the decay can enter into the inner pulp of the tooth, (nerve) causing a bad toothache. At this point root canal treatment is necessary, to remove the infected nerve. Unfortunately RCT’s are costly and very often fail after a short period of time, and if this happens the only remedy is to extract the tooth. If a patient decided to have a bridge after an extraction of a tooth following a RCT failure, then the two adjacent teeth would need to be filed down in order to fit the two crowns with the pontic unit in between, to fill the gap. This means that the strength of these teeth have to be compromised in order to fit a bridge. Bone resorption (loss) will occur as the bone fills the gap left by the missing tooth, which in turn can cause the gum under the pontic bridge unit to shrink, leaving a gap under it. Another consideration is that bridges are generally only expected to last for an average of eight years, whereas implants would be expected to last double this amount of time, and there is minimal bone resorption if an implant is placed immediately following an extraction.
